The "Overview" Table (with the ref. numbers) doesn't have a entry for "Infrastructure Configuration Management Testing".

The introduction should probably be rewritten to reflect the fact that this is a chapter in the testing guide, rather than a guide on its own.

Minor things:

• capitalization is not used consistently in the table

The term "gates" to identify entry points of the application seems very non standard. Section 4.2.3 uses "entry points".
